Get information about all the care programs and services from Southwestern Health Resources (SWHR).
Know about the staff, their qualifications and what they do.
Be told who their case manager is and how to ask for a different one if needed.
Get help from care management when making decisions with their doctors.
Choose to leave a program or service at any time.
Be treated with kindness and respect.
Have their private information kept safe and confidential.
Know how to file a complaint if they have one.
Expectations of patients:
Follow the care plan agreed upon with their case manager or let them know if they can't.
Give SWHR the information needed to provide the right services.
Tell their case manager or doctor if they leave the program.
